What is a Medical Certificate for IMT
The Medical Certificate for IMT is an official document confirming that a driver is in adequate physical and psychological condition to operate a vehicle in Portugal. It is required by the Instituto da Mobilidade e dos Transportes (IMT) when applying for or exchanging a driving licence, including converting a foreign licence into a Portuguese one. The certificate is issued after a medical assessment and is a mandatory part of the licence process.
How to get a Medical Certificate for IMT online
To obtain the certificate online, fill out the medical questionnaire on the Oladoctor website. The doctor will review your responses and, if your health meets the requirements, issue the certificate electronically. If the doctor cannot confirm your fitness to drive, the payment will be refunded.
Who can request the IMT certificate?
Anyone who:
Needs to exchange or renew a driving licence at IMT in Portugal (Portuguese or foreign).
Is 18 years or older.
Holds a valid ID card or passport (details required in the form).
Possesses a Número de Utente (National Health Service user number).
Possesses a Número de Contribuinte (Portuguese Tax Identification Number)
Has a residential address in Portugal.
Does not require an in-person exam according to IMT rules (most routine exchanges).
Note: If you have a complex medical history (e.g. uncontrolled epilepsy), book a video consultation first.
How much does the certificate cost and how do I pay?
Price: €39 (flat fee, paid once).
Payment method: Pay securely online using your bank card. Payments are processed via Stripe (international payment system). The full amount is charged at checkout — no hidden fees.
Do I need a video consultation?
No, a video call is not required.
You fill out a short online form; the doctor reviews it asynchronously. Prefer to talk first? Book a video call with Dr Alina Tsurkan or Dr Nuno Tavares Lopes — both can guide you through the process. Dr Tsurkan speaks Portuguese, Ukrainian, and Russian. Dr Lopes speaks Portuguese, English, and Spanish.
How do I request the certificate step by step?
Create or log into your Oladoctor account.
In the left-hand menu click Medical Certificates → Medical certificate for IMT.
Complete the medical form (personal data, health questions, ID number).
Pay €39 with your bank card.
Wait for doctor review. If clarification is needed, the doctor will message you in your account chat.
What happens after you submit your request?
Doctor review time: within 24 hours.
If approved:
PDF certificate is emailed to you.
The same file appears in My Account → My Orders → Download.
If the doctor cannot issue the certificate:
You get a full refund automatically.
Refund reaches your card in 5–10 business days (bank-dependent).
How long is the IMT medical certificate valid?
The IMT medical certificate is valid for 3 months from the date of issue.
How do I know my certificate is ready?
You’ll receive an email from Oladoctor with the certificate attached as a PDF.
You can also download the same document anytime from your Oladoctor account:
My Account → My Orders → Download
If you don’t see the email, check your spam or promotions folder.
Can I get the IMT medical certificate through a video consultation?
Yes. If you prefer to speak with a doctor before getting the certificate, you can book a video consultation instead of filling out the online request form.
When should I choose a video consultation?
If you want to discuss your health directly with the doctor
If you’re unsure whether you qualify for the certificate
If you have a complex medical history or questions before proceeding
If you simply prefer live communication
Who should I book?
You can book a video consultation with Dr. Alina Tsurkan or Dr. Nuno Tavares Lopes. Both are available for online appointments and can issue the IMT medical certificate during the call — if everything is medically appropriate.
Will I need to submit the form separately?
No. If you choose the video consultation route, you do not need to submit the certificate request form or pay for it separately. The doctor will issue the certificate directly if possible.
Don’t have an SNS number (Número de Utente)?
To issue the IMT medical certificate electronically, doctors usually need to attach your SNS number (Número de Utente) to the submission. If you’ve just arrived in Portugal and don’t have one yet, you can apply at your local centro de saúde (public health centre). Bring identification, your Portuguese tax number (NIF), and proof of address. Processing is free; the number is linked to you, not to a specific clinic.
If you’re an EU/EEA citizen: take your passport or national ID, your NIF, proof of address (e.g., tenancy/utility bill), and — if available — your EU citizen registration certificate (Certificado de Registo de Cidadão da União Europeia / CRUE).
If you’re a non-EU citizen: take your passport, your NIF, proof of address, and your residence document (e.g., visa sticker, entry permit, or residence card/manifest).
Once you have the SNS number, your doctor can submit the IMT medical certificate online against your record, which speeds up the driving licence exchange process.
